Job Description:
A company is looking for a Care Navigator for a 100% remote,
full-time position.
Key Responsibilities
Support current patients by answering questions and triaging them
to the appropriate resources
Document patient interactions and attend team meetings as
required
Conduct targeted patient outreach and provide feedback on
processes
Required Qualifications
3+ years of experience in a healthcare setting, particularly in
care navigation or patient support
Knowledge of healthcare terminology, especially in addiction
medicine
A passion for serving individuals with addiction and mental health
challenges
Experience with common business tools and technology
Ability to work in a fast-paced, rapidly changing environment
